REUP Method Revolutionizes Heart Transplantation: A Global Breakthrough in Donor Organ Preservation

Scientific Breakthrough in Heart Transplants

Researchers from Vanderbilt University Medical Center have developed a pioneering method for preserving donor hearts called REUP (Recovery via Ultraoxygenated Perfusion). This technique could dramatically increase the availability of viable donor organs and reshape heart transplantation worldwide.


“This is a game-changer,” said Dr. Aaron Williams. “It’s never been successfully used in heart transplants before. We expect it to be adopted globally.”

Why This Innovation Matters

Traditionally, hearts from donors after circulatory death (DCD) have been underutilized due to their fragile condition and the ethical or financial constraints of reanimation. REUP bypasses these obstacles by using a cold, oxygen-rich solution to flush the heart shortly after death — no machines or reanimation required.

  • Preserves heart viability for up to 8 hours
  • Eliminates need for expensive perfusion machines
  • Avoids ethical controversies related to donor reanimation

How the REUP Method Works

The solution used in REUP includes:

  • Red blood cells (RBCs)
  • Del Nido cardioplegia
  • Oxygen and anti-inflammatory additives

This cold perfusion halts cellular breakdown, reduces inflammation, and keeps the heart functional during transportation. Since November 2024, Vanderbilt has completed 20 successful transplants using REUP with excellent clinical outcomes.

Implications for Global Transplantation

REUP could be especially transformative in:

  • Low-resource countries lacking advanced perfusion devices
  • Regions with strict transplant ethics
  • Pediatric transplant units

Researchers also believe this method can be adapted for other organs, including the liver, kidneys, lungs, and potentially pediatric transplants.
